A woman involved in a three-vehicle collision nearly three years ago has been indicted on criminal charges in Barren County.
Heather E. Slater, 47, of Edmonton was indicted recently by a Barren County grand jury on charges of reckless homicide, two counts of second-degree assault and first-degree wanton endangerment. The indictment accuses Slater of recklessly causing the death of Karrie Selner in a Nov. 15, 2022, crash on Burkesville Road.
Selner, 62, of Edmonton was a passenger in a 2015 Dodge Caravan driven by Slater when it crossed the center line on Burkesville Road and struck two vehicles, according to Kentucky State Police.
KSP responded to the scene of the crash around 8:30 a.m. on Nov. 15, 2022. Slater’s vehicle crossed the center line and struck a 2011 Toyota Highlander being operated eastbound by Michael C. Forsythe, 43, of Princeton. Slater’s vehicle continued on, striking a third vehicle, a 2015 Chevrolet Equinox, also traveling eastbound and operated by Timothy Jackson, 62, of Winneconne, Wisconsin, according to a state police investigation.
Selner was pronounced deceased at the scene by the Barren County coroner. The operators of all three vehicles, along with a passenger in the Equinox, were transported to T.J. Samson Community Hospital, where they were treated for non-life-threatening injuries.
The indictment accuses Slater of wantonly causing serious physical injury to Timothy Jackson and his passenger, Georgia Jackson, in the crash. Slater is also accused of engaging in wanton conduct that created a substantial danger of death or serious injury to Forsyth.
